Attach a copy of the IRS Letter of exemption from federal income tax or other period of federal tax exempt status.

Project description

Please provide a detailed description of the project for which you are applying. Include statement of needs, problems addressed, description of target population, and how program will benefit participants/recipients.

 

Provide a description of project goals, measurable objectives, action plans and expected outcomes. Describe the plans for evaluation, including how success will be defined and measured.

Provide an overall project budget and an itemized budget for the grant monies you're requesting.
